Troocoo are seeking an experienced Care Planner (Registered Nurse) to join a residential aged care service.
This role is ideal for an RN who is passionate about person-centred care and enjoys ensuring residents receive high-quality, individualised support.
You will be responsible for care planning, assessments, and ongoing reviews while working closely with residents, families, and internal teams to ensure care aligns with quality and funding requirements.

Key Responsibilities
- Complete admission, interim, and ongoing assessments
- Develop and review individualised care plans
- Partner with residents and families to support choice and dignity
- Ensure care delivery meets Aged Care Quality Standards
- Liaise with classification and revenue teams to ensure accurate AN-ACC classification
- Conduct quality care evaluations
- Promote a resident-first, person-centred approach
- Current AHPRA Registered Nurse registration
- Minimum 3 years’ RN experience, preferably in aged care
- Experience in assessment and care planning
- Strong understanding of person-centred care
- NDIS Worker Screening Check (including Police Check)
- Knowledge of AN-ACC
- Understanding of Aged Care Quality Standards
- Previous care planning or leadership experience
