Key Responsibilities:
- Assessment and Care Planning: Conduct thorough assessments and create individual care plans tailored to the needs of each resident. Ensure that care plans are regularly reviewed and updated, and that residents and their families are actively involved in the process.
- Resident Focus: Maintain a resident-first approach, ensuring that care plans respect the dignity, preferences, and rights of residents. Foster a flexible, supportive environment for residents, promoting choice and independence.
- Collaboration with Classification Team: Work closely with the classification team to ensure that residents are correctly classified under relevant aged care funding models (ACFI/AC ACC), optimizing funding for each resident's care.
- Aged Care Quality Standards: Ensure compliance with the Aged Care Quality Standards by maintaining accurate documentation and continuously improving care delivery processes to meet regulatory requirements.
- Quality and Safety: Actively contribute to a culture of safety by promptly reporting incidents, hazards, and feedback, and ensuring all mandatory reporting is completed according to policy. Assist in managing resident complaints or concerns, ensuring timely resolutions.
- Ongoing Resident Evaluation: Regularly assess care outcomes and adjust care plans accordingly to improve the quality of life for residents.
- Essential:
- Current AHPRA Registered Nurse.
- A minimum of 2 years’ experience in a care planning role within a residential aged care setting.
- Demonstrated ability to develop, implement, and evaluate care plans in accordance with individual resident needs.
- Strong understanding of the Aged Care Quality Standards and the ACFI/AC ACC classification systems (desirable but not essential).
- NDIS Worker Check (including Police Check).
- Strong communication skills and a commitment to working in partnership with residents, families, and the wider healthcare team.
- Attention to detail and the ability to manage multiple priorities in a fast-paced environment.
- Empathy and understanding of the unique needs of older adults, with a focus on person-centered care.
- Ability to promote a positive and collaborative working environment.
