Location: Southern Downs (which cover: Warwick, Allora, Stanthorpe and Wallangarra)
Work Type: Permanent Part-Time 
Salary: $41.99 to 57.87 per hour + Salary Packaging

Troocoo is exclusively recruiting for a well-established and respected not-for-profit provider delivering community and home care services across the rural community setting in the Southern Downs region. We are seeking experienced Registered Nurses to support older Australians to live safely, independently, and with dignity in their own homes.

About the Role

As a Home Care Registered Nurse, you will provide clinical assessment, care planning, and ongoing case coordination for clients living in the community. You will work autonomously while collaborating closely with clients, families, care workers, GPs, and allied health professionals to deliver high-quality, person-centred care.

Key Responsibilities Clinical Care & Case Management:
  • Conduct comprehensive in-home assessments
  • Develop, implement, and review individualised care plans
  • Monitor health status and respond to clinical changes promptly
  • Provide wound care, medication management, and chronic disease support
  • Educate clients and families on health management and wellbeing
  • Maintain accurate electronic documentation in line with legislation
Coordination & Quality:
  • Coordinate services with care staff and external healthcare providers
  • Support and guide community care workers within scope
  • Ensure compliance with aged care quality standards and home care regulations
  • Contribute to continuous improvement and safe service delivery
  • Promote independence, reablement, and client choice in care planning

About You

You are a compassionate and proactive Registered Nurse who enjoys working independently in the community. You are confident in clinical decision-making, thrive on building rapport with clients, and are passionate about supporting older Australians to remain in their homes.

Key Selection Criteria

Essential:
  • Current AHPRA registration as a Registered Nurse
  • CPR certification and current driver’s licence
  • Experience in aged care, community, or chronic disease management
  • Strong clinical assessment and care planning skills
  • Excellent communication and documentation skills
  • Competency with electronic systems and Microsoft Office
  • Full working rights (no sponsorship available)
Desirable:
  • Experience in Home Care Packages or community nursing
  • Postgraduate qualifications in Gerontology, Community Health, or related field
  • Understanding of aged care quality standards and reablement models
Compliance Requirements
  • National Police Check (or ability to obtain)
  • NDIS Worker Screening Check (or ability to obtain)
  • COVID-19 and Influenza vaccinations (as required)
  • National Personal Insolvency Check
