About our client;
Country to Coast Queensland (CCQ) delivers the primary health network (PHN) program for Central Queensland, Wide Bay and Sunshine Coast on behalf of the Commonwealth Department of Health.  CCQ has an ambitious agenda to design and deliver world recognised primary health outcomes for the people in their region. Achieving this will require CCQ as a commissioning organisation to not only focus on primary health service provision, but also the environmental, social, and cultural determinants of health which support communities to live their best lives.
Their diverse region covers 161,108 km2 from Sunshine Coast to Rockhampton and across to Emerald. The population in the region is just under 1M people with around 68% living regionally or remotely and more than 28% living in the most disadvantaged quintile. 
CCQ collaborates with the community to provide exceptional primary healthcare services to reduce health inequalities for their region. CCQ has seven key priorities for health improvement: mental health, alcohol and other drugs, Aboriginal and Torres Strait Islander health, population health, health workforce, digital health and aged care.”

About the role;
CCQ have partnered with Troocoo to identify a Population Health Lead as a member of the Health & Wellbeing team, to ensure CCQ partner with health and social services organisations and builds capability in primary care workforce to promote wellness and keep people out of hospital.

As Population Health Lead, your key responsibilities will include:
  • Acting as a subject matter expert in population health including cancer screening, immunisation, maternal and child health, health promotion, disease prevention, health literacy and self-management.
  • Managing region-wide plans, projects, and programs that promote health and wellbeing for the whole community such as the CCQ Immunisation Action Plan.
  • Analysing population health research, reports, and data to identify emerging issues and priority areas of focus and provide high-level advice for stakeholders. • Use data-driven quality improvement strategies to improve primary healthcare and contribute to research and program evaluation.
  • Engaging meaningfully with consumers, communities, and healthcare staff in a culturally safe manner.
  • Preparing grant applications, oversee project bid processes, and ensure project completion within scope, budget, and quality management requirements.
  • Convene planning and consultation processes to better understand health service needs and priorities.
  • Developing and implementing innovative projects and programs with stakeholders that are evidence-based, responsive to local needs, and enhance service and system integration.
  • Gaining consensus and commitment in a complex policy and service environment.
  • Leading, managing, and developing staff to sponsor projects.
  
Through your years of experience you will bring to the organisation your;
  • Expertise in population health, health promotion, public health, and the social model of health.
  • Strong partnerships and collaboration skills, with the ability to build wide-ranging alliances and networks and identify opportunities for collaboration.
  • Experience in commissioning, including planning, research, market analysis, engagement, service co-design, procurement, provider relationship management, and service evaluation.
  • Strong problem-solving skills with the ability to research and analyze complex information and apply evidence-based principles to facilitate innovative solutions to complex health issues.
  • Strong project management skills, with the ability to use planning tools to track achievement of goals, align systems and processes, and use resources efficiently.
  • Strong communication skills, with the ability to tailor messages, articulate complex concepts, write persuasively, present clearly, and engage in a culturally safe manner.
  • Strong leadership skills, with the ability to develop strategic frameworks, drive capability development, and encourage continuous learning and reflection.
  • Qualifications in public health, epidemiology, or related fields and experience in population health.
