About our client;
Country to Coast Queensland (CCQ) delivers the primary health network (PHN) program for Central Queensland, Wide Bay, and Sunshine Coast on behalf of the Commonwealth Department of Health.  CCQ has an ambitious agenda to design and deliver world-recognised primary health outcomes for the people in their region. Achieving this will require CCQ as a commissioning organisation to not only focus on primary health service provision, but also on the environmental, social, and cultural determinants of health that support communities to live their best lives.
Their diverse region covers 161,108 km2 from Sunshine Coast to Rockhampton and across to Emerald. The population in the region is just under 1M people with around 68% living regionally or remotely and more than 28% living in the most disadvantaged quintile. 
CCQ collaborates with the community to provide exceptional primary healthcare services to reduce health inequalities for their region. CCQ has seven key priorities for health improvement: mental health, alcohol and other drugs, Aboriginal and Torres Strait Islander health, population health, health workforce, digital health and aged care.”

About the role;
CCQ have partnered with Troocoo to identify a Deputy Director Healthy Ageing and End-of-life Care, to provide strategic leadership of CCQ’s Healthy Ageing and end-of-life care programs and commissioning activities.  Older people make up more than 20 percent of CCQ community and represent a significant proportion of those admitted to hospital and with chronic illnesses.
Reporting through to the Executive Director Health and Wellbeing.  This role drives the vision with regard to Healthy Ageing and End-of-life care.  The role supports health, aged care, and social services to work in a more person-centered and integrated way.

As Deputy Director Healthy Ageing and End-of-life Care, your key responsibilities will include:
  • Leading the development, implementation, and evaluation of evidence-based strategies for older persons, palliative and end-of-life care
  • Fostering partnerships with health and social sector providers delivering services to older people
  • Overseeing care for older persons' projects and commissioning activities, working with CCQ teams and external stakeholders
  • Acting as an internal subject matter expert across social and clinical areas impacting older population health and wellbeing
  • Developing and implementing evidence-based projects and programs in partnership with stakeholders to enhance service and system integration.
  • Managing and developing staff as a direct supervisor and sponsor for commissioning projects.
Key skills required;
  • Leading the development, implementation, and evaluation of evidence-based strategies for aged and primary care, while maintaining a deep understanding of best practices, policy trends, and funding environments.
  • Developing strategic frameworks with cultural, community, political, and cross-agency perspectives, and prioritize workload and manage activity fluctuations.
  • Undertaking strategic planning, and program evaluation to promote outcomes-based commissioning.
  • Building wide-ranging alliances and networks, identifying opportunities for collaboration and integration, and establishing a reputation as a leader of engagement and reform.
  • Championing people-centered and integrated care, maintaining composure, and advocating for Aboriginal and/or Torres Strait Islander people in a culturally safe and respectful manner
  • Communicating effectively and tailoring messages to stakeholders, facilitating small and large groups, and creating opportunities for others to contribute to discussions.
  • Leading transformational change in a complex and fast-paced environment drives capability development and encourages continuous learning and reflection.
