We are seeking an experienced Clinical Case Manager (RN) to join a leading aged care provider in their home care team, near Port Macquarie.

This role is ideal for a registered nurse who enjoys clinical leadership, care coordination, and supporting quality outcomes for clients.

The Role

• Lead clinical assessments, care planning, and ongoing reviews
• Ensure care delivery aligns with the Aged Care Quality Standards
• Provide clinical guidance and support to care staff
• Support incident management, risk mitigation, and clinical governance
• Promote person-centred, consumer-directed care
• Liaise with families and external health professionals as required

About You

• Registered Nurse with current AHPRA registration
• Experience in clinical case management within aged care
• Strong knowledge of aged care standards and clinical governance
• Previous experience with home care funding - HCP, NDIS, DVA, etc. 
• Confident managing complex clinical needs
• Strong communication, leadership, and documentation skills
• NDIS Worker Check (or willingness to obtain)
