The Role
- Lead clinical assessments, care planning, and ongoing reviews
- Ensure care delivery aligns with the Aged Care Quality Standards
- Provide clinical guidance and support to care staff
- Support incident management, risk mitigation, and clinical governance
- Promote person-centred, consumer-directed care
- Liaise with families and external health professionals as required
- Registered Nurse with current AHPRA registration
- Experience in clinical case management within aged care
- Strong knowledge of aged care standards and clinical governance
- Previous experience with home care funding - HCP, NDIS, DVA, etc.
- Confident managing complex clinical needs
- Strong communication, leadership, and documentation skills
- NDIS Worker Check (or willingness to obtain)
