About the Role

We are seeking multiple experienced APS6 Complex Case Managers (Clinical) to provide specialised, person-centred support to veterans and their families. The role focuses on helping current and former Australian Defence Force personnel navigate complex wellbeing, health, rehabilitation, and social support needs.

You will work with veterans presenting with a range of challenges, including service-related injuries, mental health concerns, trauma, transition issues, and broader psychosocial stressors. Using trauma-informed, evidence-based frameworks, you will coordinate services, advocate for clients, and ensure they receive timely access to entitlements and support pathways.
  
Key Responsibilities
  • Provide end-to-end case management for veterans with complex or high-risk needs, acting as their primary point of contact.
  • Conduct intake interviews, psychosocial assessments, risk reviews, and ongoing monitoring of wellbeing.
  • Develop tailored case plans focused on capacity building, social participation, rehabilitation, and sustainable independence.
  • Support veterans to access clinical, mental health, rehabilitation, housing, financial, or community-based services.
  • Manage sensitive and sometimes distressing cases with professionalism and clinical judgment.
  • Liaise with health providers, ex-service organisations, community partners, advocates, and internal stakeholders.
  • Escalate high-risk situations to senior clinical staff and contribute to coordinated crisis responses when required.
  • Prepare high-quality case notes, reports, and stakeholder briefings.
  • Provide guidance to colleagues on veteran-specific needs, trauma impacts, and clinical considerations.
  • Contribute to continuous improvement of case management practices for the veteran community.
  
Essential Requirements
  • Tertiary qualifications in Psychology, Social Work, Occupational Therapy, Nursing, or a related clinical field.
  • Demonstrated experience providing case management or clinical support to individuals with complex, trauma-related, or co-morbid needs.
  • Proficiency with case management systems and MS Office.
  • Australian citizenship, National Police Check, and ability to obtain a Baseline Security Clearance.
  
Highly Desirable
  • Knowledge of trauma-informed practice, suicide risk considerations, and crisis intervention.
  • Familiarity with psychosocial rehabilitation, wellbeing frameworks, or support coordination models.
