Clinical Care Manager

Are you an experienced Home Care leader looking for an opportunity to make a meaningful impact?

We are seeking a Clinical Care Manager to join a respected home care provider supporting older Australians to live independently and safely in their own homes. This is a hands on leadership role focused on driving clinical excellence, quality improvement, and readiness for the upcoming Support at Home reforms.

Key responsibilities include:

• Leading clinical governance and quality improvement initiatives
• Conducting complex clinical assessments and care planning
• Managing clinical risks and supporting incident investigations
• Supporting Support at Home readiness and implementation activities
• Coordinating multidisciplinary care and external service providers
• Providing clinical leadership, mentoring and support to nursing and care teams
• Ensuring compliance with Aged Care Quality Standards and relevant funding programs

About You To be successful in this role, you will bring:

• Current AHPRA registration as a Registered Nurse
• Minimum 3 years' experience in a Home Care management role
• Strong knowledge of Home Care Packages and Support at Home reforms
• Demonstrated experience in clinical governance and quality improvement
• Experience leading clinical teams and managing complex client care
• Excellent stakeholder engagement and communication skills
• Sound understanding of aged care legislation, standards and compliance requirements
• Current First Aid and CPR certification
